This aircraft was one of two DHC-4A Caribous along with an aircraft hangar, two Twin Otters (DHC-6) gifts from CIDA (Canadian International Development Agency) to the people of Guyana.

International Aviation Ltd was hired to ferry the aircraft from Guyana St. Jean, Quebec, Canada. The aircraft crashed 48 miles SW of Bridgetown, Barbados on June 14, 1981. The aircraft or crew were never recovered. 

The following persons were crew members aboard when the aircraft was lost.

  • Dave Holland of Fort Chimo, Quebec, Canada – Chief Pilot from Air Inuit
  • Grant Davidson of Toronto, Canada – Pilot for International Aviation Ltd
  • Richard Cusson of Ottawa, Canada – Mechanical Engineer from Air Inuit.
